Sign up or log in to the SokdakSokdak booking platform.

2. Tuntun Children’s Hospital

Search for our clinic.

3. Infant/Toddler Screening

Select the service and complete your reservation.

• 1 Reservations are exclusively available online. Same-day walk-ins and phone bookings are not accepted.
• 2 Screenings are conducted on weekdays only. This service is unavailable on weekends and public holidays.
• 3 The booking window opens exactly 4 weeks in advance at 6:00 AM. Book Your Screening

Health Screening Questionnaire

Please complete this mandatory step prior to your visit for a seamless experience.

1. NHIS Website

Visit the National Health Insurance Service (NHIS) website and navigate to the Health iN portal.

2. Secure Login

Log in using the guardian’s Digital/Joint Certificate.

3. Complete the Form

Fill out the digital health questionnaire for your child.

• 1 Please submit the questionnaire online. For efficiency, we cannot accept printed paper copies at the clinic.
• 2 This policy is in place to honor appointment times and minimize the waiting period for all families. Thank you for your understanding.
• 3 When filling out the form online, you will create a designated registration number/password. Please memorize this, as you will need to provide it at the clinic!
• 4 Arriving 15 minutes prior to your scheduled appointment ensures a smooth and unhurried screening process. Go to NHIS Questionnaire

Check Your Child’s Eligibility

Verify if your child is currently due for their national health screening.

1. NHIS Website

Access the Health iN > Family Health Care section on the NHIS website.

2. Select Your Child

Click on the Child (Infant) Health Screening Information tab.

3. View Eligibility

Check your child’s screening status and eligible dates.

• 1 Screenings can strictly only be performed within your child’s designated date range. Please verify these dates beforehand.
• 2 Ensure you cross-check the valid screening dates with your child’s current age in months. Check Eligible Dates
